Privacy Policy

1. Who we are

Stem Cell Payment Plans (stemcellpaymentplans.com, the “Site”) is a financing information website for patients of Innovations Stem Cell Center (“the clinic,” “we,” “us,” or “our”). This Privacy Policy explains what information the Site collects, how we use and share it, and the choices you have. It applies only to this Site. It does not cover Alphaeon’s website or other websites we link to.

2. Information we collect

Information you give us

When you use the “Request a consultation” form, we collect:

• Your full name
• Your phone number
• Your email address
• The topic you would like to discuss, if you choose one (joints, back, or other)
• Your agreement to be contacted (the consent checkbox)

If you call or text the clinic directly, we also receive the information you share in that conversation.

Information collected automatically

• With each form submission: the date and time, the web address of the page you submitted from, and your device’s IP address. These are saved with your request in a private backup record.
• Analytics: we use Google Analytics 4, which uses cookies and similar technology to collect information such as pages viewed, how you reached the Site, device and browser type, approximate location (such as city or region), and how you interact with the page. We also record two specific actions: when a consultation form is submitted successfully, and when you click an “Apply” button that takes you to Alphaeon. These analytics events do not include the name, phone number, or email you enter in the form.
• Hosting logs: our hosting provider may keep standard server logs (such as IP address, browser type, the page requested, and the time of the request) for security and operations.

What we do not collect

This Site does not collect credit applications, Social Security numbers, dates of birth, income, bank account details, or payment card numbers. Financing applications are completed on Alphaeon’s own website (see section 6).

3. Please do not send medical information through the form

The consultation form is meant only to let the clinic know how to reach you. Please do not include your medical history, diagnoses, medications, or other detailed health information in the form. You can discuss your health with the clinic team directly during your consultation.

4. How we use your information

• To respond to your consultation request and contact you about scheduling a consultation
• To answer your questions about treatment options and financing
• To keep a record of your request and your consent to be contacted
• To understand how visitors use the Site and to improve it
• To protect the Site against spam, fraud, and misuse
• To comply with legal obligations and enforce our Terms of Use

5. Calls, texts, and emails

The consultation form asks you to check a box that reads:

I agree to be contacted by phone, text, or email by Innovations Stem Cell Center about my request. Message and data rates may apply. Reply STOP to opt out of texts. Consent is not a condition of treatment or financing.

If you check the box and submit the form, the clinic may contact you by phone call, text message, or email about your request, such as following up on your consultation, scheduling, and answering your treatment or financing questions. Message and data rates may apply, depending on your mobile plan. Consent is not a condition of treatment or financing; you can always call the clinic instead.

You can ask us to stop contacting you by phone, text, or email at any time. Just tell the person who calls you, reply STOP to any text message, reply to our email and ask us to stop, or call us at (972) 893-9849. We do not sell or share your phone number or your consent to be contacted with third parties for their own marketing.

6. Financing applications with Alphaeon

When you click “Apply with Alphaeon,” you leave this Site and go to Alphaeon’s website. Your application is collected and handled by Alphaeon and its issuing bank, Comenity Capital Bank, under their own privacy policies and terms, not this Privacy Policy. Please review Alphaeon’s privacy policy on its website before you apply. We do not send the information you enter in our consultation form to Alphaeon, and we do not receive the information you enter in your Alphaeon application through this Site.

7. How we share information

We do not sell your personal information. We share information only as described below:

• Service providers. We use companies that help us run the Site and respond to you, including our customer relationship management (CRM) and messaging platform (GoHighLevel, also called LeadConnector), which receives your form submission and may be used to send calls, texts, and emails; our website hosting provider (Hostinger); and Google, which provides Google Analytics. These providers may use the information only to provide services to us, subject to their own terms and privacy policies.
• Legal and safety reasons. We may disclose information if required by law, subpoena, or court order, or if we believe it is necessary to protect the rights, property, or safety of the clinic, our patients, or others.
• Business changes. If the clinic is involved in a merger, sale, or transfer of some or all of its business, information may be transferred as part of that transaction, subject to this Privacy Policy.
• With your permission. We may share information for other purposes if you ask us to or agree to it.

8. Cookies, analytics, and how to opt out

Google Analytics sets cookies in your browser to tell visits apart and measure how the Site is used. Google processes this data under its own policies. You can learn more at How Google uses information from sites or apps that use its services.

To opt out of Google Analytics on all websites, you can install the Google Analytics Opt-out Browser Add-on. You can also block or delete cookies in your browser settings. The Site will still work if you block analytics cookies. Some browsers offer a “Do Not Track” setting; because there is no common standard for it, the Site does not currently respond to those signals.

9. How long we keep information

We keep consultation requests and the related contact and consent records for as long as needed to respond to you, to keep a record of your consent, and to meet the clinic’s business, legal, and record-keeping needs. After that, we delete the information or keep it in a form that no longer identifies you. Analytics data is kept according to our Google Analytics data retention settings. If you become a patient, information you give the clinic as part of your care is handled under the clinic’s patient records practices, not this Site policy.

10. Security

We use reasonable measures to protect the information we collect. The Site is served over a secure (HTTPS) connection, the backup copy of form submissions is stored in a location that is not publicly accessible on the web, and access to our CRM is limited to people who need it. No website, internet transmission, or storage system is completely secure, so we cannot guarantee absolute security.

11. Your choices and requests

You may ask us to tell you what information we have about you from this Site, to correct it, or to delete it, and you may withdraw your consent to be contacted at any time. To make a request, call us or write to us using the details in section 16. We may need to confirm your identity before acting on a request, and we may keep certain information when the law requires or allows it. Depending on where you live, you may have additional privacy rights under applicable law, and we will honor them as required.

12. Children

This Site is meant for adults. It is not directed to anyone under 18, and we do not knowingly collect personal information from anyone under 18. If you believe a minor has submitted information through the Site, please contact us and we will delete it.

13. Where the Site is intended for use

The Site is operated from Texas and is intended for people in the United States. If you use it from outside the United States, your information will be processed in the United States.

14. Links to other websites

The Site links to websites we do not control, including Alphaeon’s application site. Those websites have their own privacy practices, and we are not responsible for them.

15. Changes to this Privacy Policy

We may update this Privacy Policy from time to time. When we do, we will post the new version on this page and change the effective date at the top. Changes apply from the date they are posted.
